What is a consultation report?
A consultation report is a physician’s written assessment, obtained no later than after the trial sessions and before the start of the actual guideline-based therapy. The official form is called Form 22 and is titled „Consultation Report Prior to the Start of Psychotherapy,“ in its 2014 version.
