Psychological Counseling and Health Insurance

ONLINE PSYCHOLOGICAL COUNSELING by Haegermann 3 min. reading time September 2026

Statutory health insurance does not cover psychological counseling that is not related to a medical condition. The Psychotherapy Guidelines define the scope of coverage under statutory health insurance (GKV) based on whether the treatment is related to a medical condition: Coverage is provided for treatments intended to diagnose, cure, or alleviate a medical condition. Psychotherapy is expressly excluded if „it serves solely the purpose of providing parenting, marriage, life, and sex counseling, as well as couples and family counseling.“.

The Other Side of Out-of-Pocket Payments

There is another side to this distinction. Since no health insurance provider is involved, no report or diagnosis is sent to them; only you and the counselor are involved; in the case of a licensed practitioner, the legal duty of confidentiality. There is still a record of the conversations, as there is in every medical practice. And if a disability insurance provider later asks about treatments and consultations, the wording of that question determines what must be disclosed.

Two ways to the register—one without it

Who Needs Treatment, is covered by insurance primarily in two ways: through the insurance office at a doctor's office or, if the insurance company does not provide the necessary treatment in a timely manner, through the Reimbursement Procedures with its own requirements. Anyone seeking guidance unrelated to illness must pay out of pocket, and that is exactly what I offer Counseling as a Self-Pay Service to.
